Document Application
1. Please first download and complete the “Student Transcript / Diploma Application Form” from the Academic Affairs Office forms download section.
2. Application Methods:

In-person application: Please bring your original identification documents and apply at the Registrar Section, Office of Academic Affairs, 2nd Floor, Building 1. If applying through a proxy, a letter of authorization and identification documents of both parties are required for verification.

Mail application: Please send your request by registered mail, including photocopies of two forms of identification (e.g., ID card, health insurance card, or driver’s license), a self-addressed stamped envelope (registered mail), and the processing fee (in the form of equivalent postage stamps), to:
Registrar Section, Office of Academic Affairs, Yuan Ze University, No. 135, Yuandong Rd., Zhongli Dist., Taoyuan City 320, Taiwan.

Transcript printing and payment machine: Located on the west side of the 2nd floor of Building 1. It can print Chinese transcripts (all years), English transcripts (all years), previous semester Chinese transcripts, and conduct/overall performance transcripts.

For WES (World Education Services) credential evaluation: Please attach the electronic files to be sent and include the email subject and content (usually in English) addressed to the recipient. After completion, send the email to the Registrar Section at
