Applicants are required to complete the「Yuan Ze University Academic Email Account Application Form」．
In the event of a forgotten account or password，please contact the responsible staff at the Personnel Office for assistance．

2．How to Apply for a Replacement Staff ID Badge If Lost？

Please proceed to the Service Desk of the Office of General Affairs to pay a processing fee of NT$200．
Afterward，submit one 2-inch ID photo along with a completed application form to the Personnel Office to request a replacement．
The processing time is approximately 7 working days．
