I. How to apply for the service certificate? How long is the application process? Shall the applicant apply for this certificate in person? (person in charge: Ms Wen-Ching Lin ext.2039)

(1) The Certificate of Employment Application Form can be downloaded and printed from the “Download Forms” section on the Personnel Office website.

(2) Applicants are required to complete the application form in person, or they may submit the request via email. If the applicant is unavailable, a representative may apply on their behalf.

(3) The applicant will be informed by a call when the certificate is ready for picking up (usually on the next day of filling out an application).

II. What are the required documents for the exiting procedure? (person in charge: Ms Ya-Fen Huang ext.2224)

Those who plan to leave their jobs shall report their leaves one month in advance and shall complete the exiting procedures as follows: return YZU Faculty & Staff ID Card; suspend from Civil Servant and Teacher Insurance, Labor Insurance, National Health Insurance; calculate the amounts of salary (for those who leave their jobs at the middle of a month), return the parking permit; hand over the possession of properties. The release paper will then be issued after these procedures are completed.

III. How to get the release paper? (person in charge: Ms Wen-Ching Lin ext.2039)

(1) It is required for those who plan to leave their jobs to turn in resignation one month in advance.

(2) Those who plan to leave their jobs shall download the YZU List of Exiting Procedures from the website of Personnel Office one week before the last day and then submit this form with the resignation and YZU Faculty & Staff ID Card.(Download Forms)

(3) Faculties who plan to leave their jobs before the end of the contract period shall turn in the contract in order to make changes of the contract period.

(4) The release paper will be issued through mailing or calling for pick up after the YZU List of Exiting Procedures is verified by the president.
