借用手續 Borrowing Procedures
(一) 於衛生保健組登記醫療器材借用申請切結書。
Applicants must register and complete the Medical Equipment Loan Application and Agreement Form at the Health Care Division.
(二) 抵押有效足以辨識個人身分之證件，憑職員證、學生證、身分證、汽機車駕照、健保卡、居留證、入台證等。
A valid personal identification document must be provided as collateral, such as a staff ID, student ID, national ID card, driver’s license (car or motorcycle), National Health Insurance card, residence permit, or entry permit.
(三) 借用期限：醫藥箱、冰枕、熱水袋、耳溫槍、酒精噴瓶、冰桶以一週為限，輪椅、拐杖、助行器以一個月為限，如有特殊原因無法如期歸還，應在到期日前3日內向衛保組辦理續借，續借以 1 次為原則。
Loan Period:
*First-aid kits, ice packs, hot water bottles, ear thermometers, alcohol spray bottles, and ice buckets may be borrowed for up to one week.
*Wheelchairs, crutches, and walkers may be borrowed for up to one month.
If the borrower is unable to return the equipment on time due to special circumstances, an extension must be applied for at the Health Care Division at least three days before the due date. In principle, only one extension is allowed.
未依規定辦理續借或逾時歸還者，自逾期日起停止其借用權，直至借用物歸還後至下學期始復權。
Failure to apply for an extension in accordance with regulations or returning items overdue will result in suspension of borrowing privileges from the overdue date. Borrowing rights will be reinstated in the following semester after all borrowed items have been returned.
